General Conditions & Eligibility

  • Waiting Periods:
  • 12 months: Pre-existing/chronic/gynaecological/cancer/HIV
  • 9 months: Maternity & related complications
  • 6 months: Scheduled surgical cases
  • Age Limits:
  • Principal & Spouse: 18–60 years entry; coverage to 65 years
  • Children: From birth (term baby ≥38 weeks) to 18 years (or 25 if full-time student & residing with parents)
  • Other Rules:
  • Premiums paid in advance/full for cover to be effective
  • Scheduled admissions: Pre-authorize 48 hours prior
  • All bills net of SHA rebates
  • Verification at accredited panel providers (carry ID/medical card)
  • Specialized tests/procedures require pre-authorization
